Your Rights Regarding Your Health Information
You have the following rights under HIPAA and related laws:
- Request restrictions on how we use or disclose your PHI. (We may not be able to honor all requests.)
- Request confidential communications (e.g., specify how and where we contact you).
- Access your medical records, either in paper or electronic form.
- Request amendments to your medical record if you believe it is incorrect or incomplete.
- Request an accounting of disclosures we have made of your PHI in the past six years (excluding disclosures for treatment, payment, or operations).
- Designate a personal representative (such as a legal guardian or someone with medical power of attorney).
- Request a copy of this notice at any time.
